to gain admission to private or public Hospitals and Clinics whenever necessary. Therefore, this important membership ID card should be carried with you at all times. Replacing a numbered Health Insurance ID card can be requested ONLY IN WRITING. To replace a lost or stolen AccessHMO membership ID Card, please contact us below.
